What to expect at a new patient consultation?
Dr. Brooks will sit down with you, go over your medical history, and review any available medical records that you bring in. If you are found to qualify for medical cannabis, Dr. Brooks will place your orders. The state will reach out to you via email to add yourself to the Registry and pay for the card itself (~$75 paid to the state). Turnaround time is relatively short, and once these steps are completed, patients are given a patient ID number and able to go to licensed dispensaries to purchase medical cannabis products.
What do I need to bring to my initial evaluation?
You will need to bring the fee ($200 cash for new, $130 cash if switching cannabis physicians), a valid Florida ID, and any available medical record copies that you believe support a qualifying diagnosis. Please understand that you must provide documentation of your condition at the time of your appointment.
What are some examples of qualifying diagnoses?
Cancer
Epilepsy
Glaucoma
HIV/AIDS
Post-traumatic stress disorder (PTSD)
Amyotrophic lateral sclerosis (ALS)
Crohn’s disease
Parkinson’s disease
Multiple sclerosis (MS)
Chronic non-malignant pain
Other diseases of the same class or kind as the above, including Cachexia (Wasting Syndrome), Complex Regional Pain Syndrome, Degenerative Disc Disease, Fibromyalgia, Generalized Anxiety Disorder, Herniated Disc, Migraines, Neuropathy, Osteoarthritis, Polymyalgia Rheumatica, Peripheral Vascular Disease, Rheumatoid Arthritis, Spinal Cord Injury, Ulcerative Colitis

Yes. Dr. Brooks will sit down with you for an initial appointment to determine if you qualify. If you are found not to qualify, you will not be charged for the visit.
I’ve seen Dr. Brooks and I qualify, now what?
Allow up to 5 business days and check your email for next steps. Once you’ve been entered into the Registry by Dr. Brooks, you will receive two emails from the Office of Medical Marijuana Use: one containing your Username and one containing a temporary password.

Once you have your username and password, apply for your Registry Identification card: https://knowthefactsmmj.com/patients/cards/
Once you have a patient identification number, you are free to visit dispensaries.
How often do I need to see Dr. Brooks?
While you pay the state an annual fee for the card, Florida statute requires that patients are seen by their qualifying physician every 7 months to renew orders. So, you’ll need to see Dr. Brooks every 7 months. Recertifications are $100.
